Additionally, it will save you time during your office visit, as your answers will be automatically entered into your medical record prior to your visit.

Please remember to bring your insurance cards and a list of your current medications with you to your visit.

Please be aware that co-pays and deductibles are collected at the end of each office visit.

After your first visit with us, you will have access to our PATIENT PORTAL which will allow you to securely view your important medical information from your home, smart phone, or anywhere with an internet connection.
